Print and complete and bring it with you to save time at registration.  Please do not write in League Use Only box.  Please use pen, no pencils. Please answer all questions.  Please come in your soccer gear ready to play.  Bring your soccer ball and a water bottle.  You must be accompanied by a Parent or Guardian, and they must sign an Medical Release Form/Insurance waiver.
